White to move

Defensive tactics. Move 40, so you have to reach the time control safely.
a) What is the best continuation for White?
b) Why is 40.Qd4 (played in the game) a fatal mistake?

White cannot afford to waste even a single tempo. To neutralise Black's dangerous attack, I would suggest:

1.Rde6 f2+
2.Kf1 Rh5
3.Re8 Rxh2
4.RxR+ QxR
5.Qxf2 Rh1+
6.Re2 with counter play.

Instead, White chose a bad move.

1.Qd4 QxQ+
2.RxQ f2+
3.Kf1 Rh5!
4.Rd2 g3
5.h3 Rxh3
6.gxR g2+
7.Kxg2 f1=Q+
8.Kh2 Qf4+ winning.
